The Care Platform is a formal language system for describing patient journeys and resource coordination in the context of holistic communal, regional, and national health and care service delivery in the visual notation of the human lens.
Since patient journeys and holistic health and care services often involve a multitude of service providers, the Care Platform takes a community oriented and patient centric approach and articulates service delivery within healthcare delivery ecosystems as collaborations within a network of organisations. In this context local communities, whānau, and patients are treated as active contributors to the health and care service design process.
The Resources, Events, and Agents (REA) concepts allow desired collaboration patterns to be expressed succinctly and in a visual notation that is easily understood by healthcare professionals and local community representatives.
